
Lansing, Mich. (May 4, 2009) – Mercy
Primary Care Center was honored by the National Kidney
Foundation of Michigan (NKFM) on Tuesday, April 28,
at the 11th Annual Lansing Champion of Hope Tribute
Dinner.
Mercy Primary Care Center received the Innovations
in Health Care Award for their program to eliminate
barriers to diabetes services, employ a diabetes registry,
and utilize a collaborative health care team to meet
individual needs of uninsured diabetic patients. The
organization was among 13 applicants invited to present
their noteworthy programs with a poster display at
the Champion of Hope Tribute Dinner at the Kellogg
Hotel and Conference Center.
The NKFM acknowledged programs that improve the lives
of Michigan residents with diabetes, kidney or cardiovascular
disease. Mercy Health Care Center was one of
three applicants who received the Innovations in Health
Care Award at the tribute dinner.
“We were very impressed by the quality and creativity
of the innovative health care presentations.” said
Daniel Carney, President and CEO of NKFM.
Mercy Primary Care Center’s winning program was “Improving
Outcomes in Uninsured Diabetics through Increased Patient
Engagement & Access to Health Care.” The
program has effectively improved patient health outcomes
by decreasing hemoglobin, increasing those who receive
eye exams, providing LDL kidney function testing and
foot exams, and increasing patient engagement.
The NKFM also partnered with the Michigan Association
of Health Plans, Michigan Health & Hospital Association,
Michigan Osteopathic Association, Michigan Primary
Care Association, and Michigan State Medical Society
to acknowledge the applicants.
The mission of the National Kidney Foundation of Michigan
is to prevent kidney disease and improve the quality
of life for those living with it.
